Talk with a care manager today
Our care managers are here for you. Care managers are nurses and mental health clinicians. They understand your health conditions and can help you get the care you need.
You can call Member Services at 1-855-242-0802 (TTY: 711). We’re here for you 24 hours a day, 7 days a week.
Care managers work with both adults and children. They share info about your health conditions and ways to take care of yourself. They can also help you:
Find a primary care physician (PCP)
Learn how to schedule doctors’ visits
Find specialists and behavioral health services near your home
Arrange rides to your provider, pharmacy or the hospital
Learn how to get medical and dental services, supplies and equipment
Find community resources and educational materials
Get the services you need
To help decide which services best meet your needs, the care manager works with:
Usted
Your provider
Your representative or guardian (if applicable)
During COVID-19, your care manager will work with you by phone. They’ll ask you about your health and care needs. You can have family and others present, if you wish them to take part.
Some health issues need more care. If you’re managing a chronic condition, your care manager can help you get the treatment you need to feel better. They can help you manage conditions like asthma, diabetes, chronic obstructive pulmonary disease (COPD) and more.
Are you planning to start a family or expecting a child? Your care manager can give you special attention if you have a high-risk pregnancy. You’ll get support for a healthier pregnancy, including info about preterm labor and birth.
